基于流式细胞检测技术的四项呼吸道病毒病原谱研究摘要目的:为了探索一种易于操作、测量指标多且针对性强、结果准确可靠的呼吸道病毒的检测和临床辅助诊断新方法。同时,通过对所收集临床病例的病原体谱结果的回顾性分析,为科学有效地防控疫情及新发病例的诊治提供依据。方法:按照纳入排除标准收集364例临床标本作为研究对象,用评价试剂(流式细胞法)和“金标准”病毒分离培养鉴定法(以下简称对比方法)同时检测同一标本中的甲型流感病毒(Influenza Virus A,IFVA)、乙型流感病毒(nfluenza Virus B,IFVB)、呼吸道合胞病毒(Respiratory Syncytial Virus,RSV)、腺病毒(Adenovirus,ADV)抗原。对两法检测结果进行比较,评估评价试剂检测的灵敏度、特异度以及两种方法检测结果的总符合率和一致性。同时使用SPSS软件对评价试剂检测结果按照年龄、性别进行分组分析。结果:两种方法对四种病毒检测结果均高度一致,检测性能基本等效。与“金标准”方法相比,评价试剂检测的灵敏度、特异度均较高,且特异性高,该法在同时检测四种呼吸道病毒方面具有良好的检测性能。364例临床标本中共检出阳性标本153例,阳性检出率为42.03%。单一病毒感染中,IFVB检出率最高,其次是IFVA、ADV、RSV:两种病毒混合感染中,以FVA为优势病毒:除单一RSV感染,男女两性的阳性检出率差异有统计学意义外,其他病毒检出率在不同性别受试者之间比较,差异均无统计学意义:病毒感染阳性患者主要来自05岁和5~21岁组人群,但各年龄组的病毒阳性检出率差异均无统计学意义。结论:流式细胞术具有测量指标多、省时省力、结果可靠的优点,适合临床辅助诊断。在呼吸道病毒感染早期,应进行及时的诊断治疗来改善疾病相关结局。同时,应注意多种病毒混合感染以及不同性别、年龄组常见病原体的差异。关键词:呼吸道病毒;流式细胞术;病毒分离:一致性:病原谱AbstractObjective:In order to explore a new method for the detection and clinical auxiliarydiagnosis of respiratory viruses,which is easy to operate,has many measurement indicatorsand strong targeted,accurate and reliable results.At the same time,through the retrospectiveanalysis of the pathogen spectrum results of the clinical cases collected,the basis for scientificand effective prevention and control of the epidemic and the diagnosis and treatment of newcases was provided.Methods:According to the inclusion and exclusion criteria,364 clinical specimens werecollected as the research objects.The influenza A virus (IFVA),influenza B virus (IFVB),respiratory syncytial virus(RSV)and adenovirus(ADV)antigens in the same specimen weredetected by the evaluation reagent (flow cytometry)and the "gold standard"virus isolationand culture identification method (referred to as the comparison method).In order to evaluatethe sensitivity and specificity of the evaluation reagent,as well as the total coincidence rateand consistency of the results of the two methods,the results of the two methods werecompared.Meanwhile,SPSS software was used to analyze the test results of the evaluationreagents according to age and gender groups.Results:The results of the two methods were highly consistent for the detection of thefour viruses,and the detection performance was basically equivalent.Compared with the"gold standard"method,the sensitivity and specificity of the evaluation reagent detection arehigher,and the specificity of the method is high.The method has good detection performancein the detection of four respiratory viruses at the same time.A total of 153 positive sampleswere detected in 364 clinical specimens,and the positive detection rate was 42.03%.Thedetection rate of IFVB was the highest in single virus infection,followed by IFVA,ADV andRSV.IFVA was the dominant virus in co-infection of the two viruses.There was nosignificant difference in the detection rate of other viruses between male and female subjectsexcept RSV infection.The virus positive patients mainly came from the 0-5 years old and5-21 years old groups,but there was no significant difference in
